# Change readiness survey

Identify what people need before adopting a planned change.

If nothing was unclear or difficult, say so. Skip questions about steps you did not experience; use not applicable where appropriate.

## 1. How clear is the reason for the change?

Answer: ____________________

## 2. Which task are you unsure how to perform afterward?

Answer: ____________________

## 3. What support would help you prepare?

Answer: ____________________

## 4. Which role needs more specific preparation?

Answer: ____________________

## 5. Where would you look for the final rollout instructions?

Answer: ____________________

## Author notes / illustrative keys where applicable

1. Check whether people can explain the change's purpose and intended effect on their work. Understanding the reason remains separate from agreeing with the decision.
2. Locate the task that people cannot yet picture after rollout. Provide task-specific preparation rather than assume a general announcement establishes readiness to use the new process.
3. Identify the missing resource, practice or support route needed before rollout. Assign preparation responsibilities without treating every request as evidence that the change should stop.
4. Find a role whose changed responsibilities are insufficiently explained. Tailor the preparation to that role instead of distributing identical instructions across unaffected teams.
5. Check which location people regard as the final rollout source. Establish a maintained instruction set so outdated drafts do not become competing routes for action.

Interpretation caution: Readiness ratings do not guarantee adoption or business impact.
