# Meeting effectiveness survey

Reduce meetings that lack a clear decision or owner.

If nothing was unclear or difficult, say so. Skip questions about steps you did not experience; use not applicable where appropriate.

## 1. Did this meeting have a clear purpose?

Answer: ____________________

## 2. Which part could have been handled before the meeting?

Answer: ____________________

## 3. Are the next actions and their owners clear?

Answer: ____________________

## 4. Which decision was made during the meeting?

Answer: ____________________

## 5. What information would have helped you prepare beforehand?

Answer: ____________________

## Author notes / illustrative keys where applicable

1. Check whether participants understood the meeting's purpose before judging its usefulness.
2. Identify preparation that could have happened earlier; do not presume all discussion belongs in a document.
3. Test whether the next actions have identifiable owners and dates rather than relying on a general feeling of alignment.
4. Record the actual decision so discussion, updates, and decision-making are not counted as the same outcome.
5. Find an input that would have changed preparation; more advance material is not necessarily more useful material.

Interpretation caution: Distinguish informational meetings from decision meetings when interpreting ratings.
