# Mentoring match feedback survey

Check whether mentoring pairs share useful goals.

If nothing was unclear or difficult, say so. Skip questions about steps you did not experience; use not applicable where appropriate.

## 3. What adjustment would improve the match?

## Author notes / illustrative keys where applicable

1. The desired help identifies the respondent's mentoring goal. Compare it with the support the pair can actually offer rather than assuming meeting frequency is the main problem.
2. A useful meeting identifies the mentoring activity that supported a concrete next step. Compare its purpose with the pair's goals, without treating enjoyment or regular attendance as evidence of mentoring effectiveness.
3. A proposed adjustment identifies how the pair could better align goals, expectations or practical arrangements. Agree the change with both people before testing it.

Interpretation caution: A mismatch does not imply poor mentoring.
