1. Ask team members to describe the agreed change, rather than judging an individual owner. A disagreement about what was agreed suggests the action record needs clarification before progress can be reviewed.
2. Identify which progress fact was missing: current stage, next check, unresolved blocker or decision owner. These require different updates and should not be collapsed into a general communication rating.
3. Ask what evidence would help the team decide whether to continue, revise or stop the action. Use the answer to set a review question that addresses the original feedback finding.

Interpretation caution: Do not imply changes caused outcomes.


## Review an action against the original feedback

A completed task and an improved experience are different observations.

### Choose the change

Bounded test: Name the specific feedback pattern and the one process adjustment being tested.

Review condition: Keep unrelated suggestions outside this test.

### Check delivery

Bounded test: Confirm the change reached the team members who should encounter it.

Review condition: A task marked done does not establish exposure.

### Revisit experience

Bounded test: Ask those members about the original difficulty and remaining friction.

Review condition: Record participant context rather than claiming causation from fewer comments.

## Collection protocol

When and whom to ask: Ask participating team members at the agreed action-review date after showing the shared action record and any available progress evidence. Do not ask them to evaluate a named colleague.

Decision owner: The team facilitator and the owner of the shared feedback-action record.

Follow-up: At the newly recorded review date, ask whether the team can identify the agreed action, current evidence and next decision. Note unresolved blockers and make a continue, revise or stop decision; observed improvements do not establish that the action caused an outcome.

Response handling: Collect feedback on the action and review process, not named employee performance. Summarize themes and allow participants to omit sensitive context.
