# Task usability survey

Evaluate one workflow rather than asking whether an entire app is easy.

If nothing was unclear or difficult, say so. Skip questions about steps you did not experience; use not applicable where appropriate.

## 1. Were you able to finish the task you intended?

Answer: ____________________

## 2. Which step was hardest to understand?

Answer: ____________________

## 3. What did you expect to happen at that step?

Answer: ____________________

## 4. Which label or instruction did you interpret differently?

Answer: ____________________

## 5. What did you do when you became unsure?

Answer: ____________________

## Author notes / illustrative keys where applicable

1. Distinguish completed, partly completed, and abandoned tasks rather than measuring confidence alone.
2. Locate the unclear step so a usability revision has a specific target.
3. Compare expected and observed behavior before deciding whether the interface or instruction needs changing.
4. Capture a label's interpretation without implying that the respondent should already know the product's terminology.
5. Learn the recovery route the person tried; assistance-seeking is not automatically evidence of failed usability.

Interpretation caution: Separate observed task success from self-reported ease.
