# Workshop learning feedback survey

Check whether a hands-on session supported practice.

If nothing was unclear or difficult, say so. Skip questions about steps you did not experience; use not applicable where appropriate.

## 1. How clear were the activity instructions?

Answer: ____________________

## 2. Which activity would you use again outside the workshop?

Answer: ____________________

## 3. What additional practice would you need?

Answer: ____________________

## 4. Was enough time available for hands-on practice?

Answer: ____________________

## 5. Which part of the activity would you repeat with support?

Answer: ____________________

## Author notes / illustrative keys where applicable

1. Check whether participants could begin the intended activity using the instructions provided. Clarify the confusing step rather than assuming an unfinished exercise reflects poor motivation.
2. Find an activity participants can connect to their own setting, then investigate what adaptation it requires. Intended reuse remains separate from observed use after the workshop.
3. Identify the part needing another attempt and the conditions for practicing it. Choose targeted rehearsal instead of repeating the entire workshop without locating the difficulty.
4. Compare the time participants needed to attempt the exercise with the time available. Separate instruction delays from insufficient practice time before extending the session.
5. Locate an activity segment where guided repetition would help. The facilitator can supply support at that point rather than infer that all participants need equal assistance.

Interpretation caution: Enjoyment and ability to apply a skill should be asked separately.
